What Is USPS Form 1583

Before we can legally receive and manage mail on your behalf, the U.S. Postal Service requires you to fill out Form 1583.
This form gives us permission to accept mail and packages in your name or your company’s name.

Here’s what you need to complete it:

  • Two forms of ID (one must be a photo ID)
  • A notarized signature — we offer this step virtually, or you can do it in person
  • The business or personal names you want your mail to arrive under
